29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  a) Jobsite, 941 Miami Avenue, Pittsburgh, PA:  On or about February 27, 2019, the employer did not ensure a safe means of accessing the ladder jack and roof work areas because the pick was set up on the outside of the ladder supports, causing employees to climb up and over the pick to get up and down, exposing them to a fall hazard of approximately 15 feet to the ground below.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use:  a) Jobsite, 941 Miami Avenue, Pittsburgh, PA:  On or about February 27, 2019, the employer did not ensure the ladders used for access and egress to the work areas were properly inspected for defects.  Several of the ladders had bent/damaged rungs and showed evidence of cracking.

The employer told the csho that he had only trained two of the employees.
