29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) Tire Center - On or about March 1, 2019, the employer failed to ensure an energy control procedure was developed, documented, and utilized for the control of hazardous energy when an employee was engaged in a service and maintenance activity when replacing the right rear tire on a semi-truck trailer.  An employee was injured when the customer unexpectedly entered the idling truck and began to pull it forward, which caused the trailer to slip off of the jack and come down onto the employee's right hand middle and index fingers and rear tire.  The employee was thereby exposed to the hazards associated with the trailer's unexpected movement and gravitational energy.
