OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm, in that employees were exposed to amputation/laceration hazards: a) Plant - On or about 02/10/19 and at times prior thereto the outdoor inclined conveyor for the chipper was not guarded, exposing workers to unprotected ingoing nip points created by the rollers, the conveyor band, and the fixed parts of the structure. b) Plant - On or about 02/10/19 and at times prior thereto the conveyor connecting the Tri State saw and the Newman KM-16 trim saw was not guarded, exposing workers to unprotected ingoing nip points created by the rollers, the conveyor band, and the fixed parts of the structure. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

The shaft end did not present a smooth edge, and projected more than half a diameter. b) Plant - On or about 02/20/19 and at times prior thereto the shaft in the chain conveyor placed at the outlet of the Pendu rip saw was not guarded, and projected more than one diameter. c) Plant - On or about 02/20/19 and at times prior thereto the conveyor connecting the Tri State saw and the KM-16 trim saw had an unguarded shaft which did not present a smooth edge, and projected one diameter. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET). Lincoln Lumber Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.219(c)(4)(i), which was contained in OSHA inspection number 1072029, citation number 1, item number 2-a, and was affirmed as a final order on 10/02/2015, with respect to a workplace located at 414 County Farm Road, Brookhaven, MS 39602.
