29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:   This violation occurred on or about March 7, 2019, at the job site located at 241 Giddings, Forney, TX; where employees using Werner extension ladders that did not extend 3-feeet above the landing of the roof and exposing employees to a fall hazard.  (a)   24-feet extension ladder, model number D1224-2, which was located at the front of the house.  (b)  28-feet extension ladder, serial number 5175101218, which was located on the side of the house.
