29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.       On or about 28 February 2019, and at times prior thereto, the employer did not develop and implement documented procedures for the operation, feeding of material, and clearing of jams for the Bradbury Vee Panel machine, exposing employees performing the activities to the hazard of being caught in the rollers.     Berridge Manufacturing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1150254, citation number 01, item number 002 and was affirmed as a final order on 08/23/2016, with respect to a workplace located at 2201 Rudeloff Road, Seguin, TX 78155.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch.     On or about 7 March 2019, and at times prior thereto, the Baldor 6-Inch Bench Grinder mounted on a pedestal had a tongue guard that was not adjusted to within 1/4-inch from the surface of the abrasive wheel, exposing the employees utilizing the grinder to the hazard of being struck by the rotating wheel or flying projectiles from a defective wheel.    Berridge Manufacturing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(b)(9), which was contained in OSHA inspection number 1150254, citation number 01, item number 007 and was affirmed as a final order on 08/23/2016, with respect to a workplace located at 2201 Rudeloff Road, Seguin, TX 78155.

On or about 7 March 2019, and at times prior thereto, a painter required to wear a Honeywell North 7600 Full Facepiece Respirator was not fit tested annually, exposing the employee to respiratory hazards associated with enamel based paints.
