29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms and walking-working surfaces were not kept clean, orderly and in sanitary condition:    a. Planetarium Server Room - On or about March 13, 2019, employees were required to enter and work in the server room during server troubleshooting activities, and the employer failed to ensure that the area was maintained in a clean and sanitary condition.  Black mold growth was found on walls and other surfaces.

29 CFR 1904.29(b)(3): Each recordable injury or illness on the OSHA 300 Log and 301 Incident Report was not recorded within seven (7) calendar days of receiving information that a recordable injury or illness has occurred:  a. Site - On or about January 12, 2019, an employee experienced a laceration which resulted in medical treatment beyond first-aid, and the employer failed to record the injury/illness on an OSHA Form 300 within 7 calendar days.
