(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: On the NPK Platform, an employee was exposed to a 6 ft. fall hazard while using a hand-held remote control to probe and allow for the removal of soybeans inside the rail car.

29 CFR 1910.30(a)(3)(iv): (iv) The correct use of personal fall protection systems and equipment specified in paragraph (a)(1) of this section, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer. On March 13, 2019, at the rail way tracks, an employee standing on top of the rail car with the self-retracting lifeline attached to the employee's front D-ring, instead of the back D-ring as prescribed in the manual for the harness for fall arrest protection.
