29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: a) West side of building, south end, on or about 3-14-19: Four employees were accessing and exiting a scaffold platform while climbing over the ladder jack brackets, exposing the employees to a fall hazard of 14 feet 4 inches. b) West side of building, south end, on or about 3-14-19: Four employees were accessing the roof surface from a scaffold platform and exiting the roof surface onto the scaffold platform, with a change in elevation that was greater than 24 inches, exposing employees to a fall hazard of 14 feet 4 inches. ABATEMENT CERTIFICATION REQUIRED FOR THIS ITEM
