29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    On or about March 12,  2019 at Southern Gear and Production, LLC the employer failed to ensure that a Lockout/Tag out program was developed for the following equipment:     1) Mazak Quick Turn QTN - 20/30   2) Mazak Quick Turn QT - 25 C/L    Employees would regularly perform preventative maintenance on the machines.

On or about March 12, 2019 at Southern Gear and Production, LLC the employer allowed employees to use compressed air for cleaning shavings off of parts.    Instance 1) at the Yamazaki lathe the air pressure was 40 psi  Instance 2) at the Mazak VTC the air pressure was 60 psi

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.    On or about March 12,  2019 at Southern Gear and Production, LLC the employer failed to ensure that the old electric panel was effectively closed. Two knockouts were missing. Employees went into the electric panel to turn the power off at the breaker for equipment in the machine shop.
