At the jobsite, employees were not wearing hardhats while working below other employees that were roofing a two story residential house, exposing employees to struck-by hazards from falling objects.

At the jobsite, employees operating Bostitch nail guns while performing roofing activities were not wearing eye protection, exposing employees to the hazard of being struck by flying objects.

At the jobsite, employees engaged in roofing activities at a height of over 6 feet were not protected by a fall arrest system. Employees were exposed to an unprotected fall of over 16 feet to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access;    At the jobsite, employees engaged in roofing activities work were exposed to fall hazards while using a ladder that did not extend at least three feet above the roof's landing.
