29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: (a) Worksite: On or about 3/18/2019, the employer did not ensure that employees worked from the deck of an elevated lift platform, where they were protected from fall hazards by a guardrail system. Employees climbed out of the elevated lift platform and worked from above it's guardrails, where they were exposed to a fall hazard of approximately twenty-one feet.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated. (a) Worksite: On or about 3/18/2019, the employer did not ensure that an employee was protected from fall hazards while working from an aerial lift at a height of approximately fourteen feet above the lower level.

29 CFR 1926.758(g): Purlins and girts were used as an anchorage point for fall arrest systems without written approval from a qualified person: (a) Worksite: On or about 3/18/2019, the employer did not ensure that a purlin being used by two employees as a fall arrest anchorage point, at a height of up to twenty-two feet above the lower level, would support at least 5,000 pounds per employee.
