(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:      - Guardrail systems [per 29 CFR 1910.28(b)(1)(i)(A)];   - Safety net systems [per 1910.28(b)(1)(i)(B)]; or   - Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems [per 29 CFR 1910.28(b)(1)(i)(C)]    Seven Bridges Ice Arena, south, compressor room - on or about March 11, 2019, employees assigned to refill and clean overhead Calcium Chloride tanks were exposed to fall hazards up to 15 feet when climbing structures and standing on the unguarded platforms without fall protection and/or fall prevention systems provided. 29 CFR 1910.23(b)(9): The employer must ensure ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury;    Seven Bridges Ice Arena, south, compressor room - on or about March 11, 2019, employees assigned to refill and clean overhead Calcium Chloride tanks were exposed to fall hazards when ascending/descending a step ladder to gain access to the platforms. Subject step ladder was not inspected to identify a missing left rail's metal spreader and to recognize its unsafe use as an extension ladder with its rear rails suspended off the ground and not locked in the open position to ensure intended balance.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    Seven Bridges Ice Arena, south, compressor room: on or about March 11, 2019, the employer did not provide personal protective equipment for employees assigned to refill and clean overhead Calcium Chloride tanks. Employees had their skin and eyes exposed to contact with Calcium Chloride 32% likely to result in irritation. 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    (a) Operations office, two relocatable power taps and Husky extension cord - on or about March 18, 2019, employees were exposed to fire and electrical hazards where an energized relocatable power tap was used as a substitute for fixed wiring of electronic devices. Black relocatable power tap was not directly connected to a permanently installed branch-circuit receptacle outlet and was connected in-series to an extension cord and another white relocatable power tap. This equipment is not intended for such use.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).      (b) Maintenance area/dock, relocatable power tap and extension cord - on or about March 18, 2019, employees were exposed to fire and electrical hazards where an energized relocatable power tap was used in an industrial setting as a substitute for fixed wiring of electronic devices. 29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following:     - A list of the hazardous chemicals known to be present using a product identifier that is referenced on the appropriate safety data sheet (the list may be compiled for the workplace as a whole or for individual work areas) per 29 CFR 1910.1200(e)(1)(i); and,     - The methods the employer will use to inform employees of the hazards of non-routine tasks (for example, the cleaning of reactor vessels), and the hazards associated with chemicals contained in unlabeled pipes in their work areas per 29 CFR 1910.1200(e)(1)(ii). 29 CFR 1910.22(a)(2): Surface Conditions - The employer must ensure the floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition.  When wet processes are used, drainage must be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided.   Seven Bridges Ice Arena, south, compressor room: on or about March 18, 2019, employees working in the compressor room were exposed to slip-and-fall hazards due to wet floor in the main walkway from equipment leaks. Adequate floor drainage and/or other controls were not implemented to prevent leaks and/or remove water from the area.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

Each employer required by this Part to keep records of fatalities, injuries, and illnesses must record each fatality, injury and illness that:    1. is work-related per 29 CFR 1904.4(a)(1); and,    2. is a new case per 29 CFR 1904.4(a)(2); and,    3. meets one or more of the general recording criteria of �1904.7 or the application to specific cases of ��1904.8 through 1904.12 per 29 CFR 1904(a)(3).    On or about March 18, 2019, the employer failed to maintain required OSHA 300 form "Log of Work-Related Injuries and Illnesses" and did not record a work-related injury that occurred on February 20, 2019.    There is no abatement certification or documentation required for this item.
