29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A form or equivalent:  Company Wide - The Employer did not maintain the OSHA 300 form or equivalent to record work-related illnesses and injuries, and the OSHA 300A form did not contain establishment information, employment information, and signature.  Worcester Wreath Company was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1245321, citation 2, item 1 and was affirmed as a final order on September 12, 2017, with respect to a workplace located at 1013 North Street, Harrington Maine.
