On or about March 18, 2019, at the jobsite, employees installing wall outlets and electrical breakers were exposed to electrical contact hazards, in that, they were not provided a Lock Out Tag Out procedure that identified the power sources and isolation steps for the 120 volt circuits that were being worked on.

On or about March 18, 2019, at the jobsite, employees involved in installing wall outlets, overhead lighting and electrical breakers were exposed to 120 volt electrical shock hazards, in that, the Lock Out Tag Out training employees received did not establish procedures how to identify and isolate electrical energy sources at the breaker panels and the equipment required to tag out the breakers.

29 CFR 1915.82(b)(1): Lights with bulbs that are not completely recessed were not equipped with guards to prevent accidental contact with the bulb:  a.  On or about 3/18/19, in the staircase and basement of the Crawford building, the overhead florescent bulbs were not protected from accidental contact, in that, the bulb covers were missing.
