29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:   a. Waksman Institute of Microbiology Addition - Hoes Lane W., Piscataway, NJ  The employer authorized the use of an attachment to a JLG, Model G6-42A, S/N 0160016555, all-terrain forklift that was not designed specifically by the manufacturer for use with the equipment.  Employees engaged in glazing activities worked from a work platform, attached to the forks, that was not authorized for use by the manufacturer.   Violation observed or about March 19, 2019

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:   a. Waksman Institute of Microbiology Addition - Hoes Lane W., Piscataway, NJ  The employer did not ensure that employees elevated in a JLG, Model G6-42A, S/N 0160016555, all-terrain forklift were protected from falls.  Employees engaged in glazier activities were working from a basket (scaffold platform) attached to an all- terrain forklift without the use of a personal fall arrest system.   Violation observed on or about March 19, 2019
