29 CFR 1926.501(b)(4): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:       a) Access way between center building and northernmost building, second and third floors, on or about 3-19-19:  Employees were using an unprotected stair opening for ladder access to upper floors, without fall protection, exposing the employees to a fall hazard of 9 feet 6inches from floor 3 to floor 2 and 22 feet 6 inches from floor 2 to the basement below.       ABATEMENT CERTIFICATION MUST BE SUBMITTED FOR THIS ITEM

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):        a) Northeast corner of building complex, south panel of roof, on or about 3-19-19:  Three employees were working on a sloped roof (4:12 pitch) and were not protected from falls, exposing the employees to a fall hazard of 40 feet 2 inches from the eave to the ground below.       b) Northeast corner of building complex, south panel of roof, on or about 3-19-19:  Three employees were working on a sloped roof (4:12 pitch) and were not protected from falls, exposing the employees to a fall hazard of up to 52 feet 2 inches from the gable end to the ground below.       c) Northeast corner of building complex, south panel of roof, on or about 3-19-19:  Three employees were working on a sloped roof (4:12 pitch) and were not protected from falls, exposing the employees to a fall hazard of up to 20 feet through trusses from the sheathing to the third floor deck below.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    a) Northeast corner of building complex, south panel of roof, on or about 3-19-19:  Three employees were working on a sloped roof (4:12 pitch) and were not protected from falls, in that they could fall more than 6 feet due to excess slack in their safety ropes, exposing the employees to a fall hazard of 40 feet 2 inches from the eave to the ground below.     b) Northeast corner of building complex, south panel of roof, on or about 3-19-19:  Three employees were working on a sloped roof (4:12 pitch) and were not protected from falls, in that they could fall more than 6 feet due to excess slack in their safety ropes, exposing the employees to a fall hazard of up to 52 feet 2 inches from the gable end to the ground below.     c) Northeast corner of building complex, south panel of roof, on or about 3-19-19:  Three employees were working on a sloped roof (4:12 pitch) and were not protected from falls, in that they could fall more than 6 feet due to excess slack in their safety ropes, exposing the employees to a fall hazard of up to 20 feet through trusses from the sheathing to the third floor deck below.
