29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired:    a) 2036 Buttonwood Rd, Berwyn, PA - The employer allowed the employee to use a 24 foot Werner extension which had defective components. The ladder was missing the rubber grips on it's shoes thus causing the ladder to displace and allowed the employee to fall approximately 20 feet to the ground. This condition was noted on or about March 15, 2019.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:    a. 2036 Buttonwood Road, Berwyn, PA 19312 - The employer did not provide employees with training on the proper placement and handling of portable extension ladders and how to recognize hazards associated with ladders. While an employee was descending a portable extension ladder from a higher level, the ladder slip out from underneath him.  This employee sustained serious bone fractures and back injuries.  Observed On or About February 26, 2019

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.    a. Detached garage, 2036 Buttonwood Road, Berwyn, PA 19312 - While an employee was descending a portable ladder, the ladder slip out from underneath him.   This employee hospitalized and sustained serious bone fractures and back injuries.  The employer failed to report a work-related, in-patient hospitalization within 24 hours of the incident. Observed On or About February 26, 2019
