29 CFR 1910.28(b)(3)(i): (3) Holes.  The employer must ensure: (i) Each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following:    a. On or about March 20, 2019, on a 2:12 pitch metal roof, an employee performing roof repairs was not protected from a fall hazard by use of a personal fall arrest system, guardrail system or cover, exposing himself to a 14-foot fall hazard. The employee fell through the damaged roof to the interior of the structure, resulting in a fatality.
