On or about 3/22/19.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.1030(c)(1)(ii)(B): The Employer's Exposure Control Plan did not contain the schedule and method of implementation for paragraphs (d) Methods of Compliance, (f) Hepatitis B Vaccination and (g) Communication of Hazards to Employees.  a) Main Medical Area: The employer's Exposure Control Plan did not include site specific information for items such as but not limited to Personal Protective Equipment, Engineering and work practice controls, Regulated Waste and Hepatitis B provisions and the Bloodborne pathogen training program. On or about 3/22/19.                          NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM.
