29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  (a)  On or about March 19, 2019, employees were exposed to injuries resulting from falls in excess of 20 feet, while removing roofing shingles from the roof of a residential building from JLG Industries, Inc., model # 660SJ, serial # 604289 and 876914, aerial lifts, with an articulating boom without the benefit of a fall restraint system being in place.
