29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operators and other employees from struck-by and laceration hazards during the use of a table saw:      (a)Alcom, LLC, at 501 East 52nd Street North, Sioux Falls, SD 57104: On or about and at times prior to March 20, 2019, the employer did not ensure employees were protected from struck-by hazards when operating the JET JPS-10TS table saw to cut both aluminum and wood in that the anti-kickback component of the guard was not installed. This condition exposed employees to struck-by hazards.

29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zone during operating cycles:     (a)Alcom, LLC, at 501 East 52nd Street North, Sioux Falls, SD 57104: On or about and at times prior to March 20, 2019, the employer did not ensure employees were protected from amputation hazards in that the front guard on the mechanical shear was not adjusted appropriately.  The opening between the work surface and the guard was greater than 0.625 inches with the point-of-operation being less than 6 inches from the face of the guard. This condition exposed employees to amputation hazards.

This condition exposed employees to amputation hazards.
