29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    a) 207 N. Main Street Farmington, DE.- On or about March 27, 2019, the employer failed to ensure that the developed energy control procedures clearly identified specific steps to be followed to control hazardous energy for equipment (CNC Machines) on which employees perform servicing, installing and removing dies, and cleaning.    NOTE: The procedures must be documented in sufficient detail and provide enough direction so that employees can effectively follow the procedure and determine how to safely perform the servicing and maintenance activities. Over generalization can result in a document which has little or no utility to the employee who must follow procedure. However, while the procedure is required to be written in detail, this does not mean that a separate procedure must be written for each and every machine or piece of equipment. Similar machines and/or equipment (those using the same type and magnitude energy) that has the same or similar types of controls can be covered with a single procedure.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:     a) 207 N. Main Street Farmington, DE - On or about March 27, 2019, the employer failed to ensure a review of the developed written energy control procedure was conducted at least annually.     Abatement Note:      1.  Where lockout should be used for energy control, the periodic inspection shall include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected.1910.147(c)(6)(i)(D)     2.  Where tagout  should be used for energy control, the periodic inspection shall include a review, between the inspector and each authorized and affected employee, of that employee's responsibilities under the energy control procedure being inspected, and the elements set forth in paragraph (c)(7)(ii) of this section.1910.147(c)(6)(ii)     3.  The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.

The synthetic web slings are utilized for hosting/lifting materials, such as dies, with a jib and/or gantry cranes to hoist materials including dies. The employer failed to ensure synthetic web slings were inspected for damage or defects by a competent person.    b) 207 N.Main Street Farmington, DE - On or about March 27, 2019 an alloy steel chain was utilized for lifting with the jib and gantry crane to hoist material include dies sets. The alloy steel chain was not inspected for damage prior to use for defects by the employer or a competent person designated by the employer.
