29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support): (a) Worksite: On or about 3/26/2019, an employee was exposed to fall hazards while ascending and descending an extension ladder that was leaning against a wall approximately fourteen feet in height, with the base of the ladder approximately sixteen feet in distance from the face of the wall, instead of maintaining a 4 to 1 ratio, which would have positioned the base of the ladder between three and four feet off the face of the wall.
