29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  Trench, 914 Pleasant Street, Amherst, MA: on or about March 14, 2019 employees were not adequately protected from cave-ins by an adequate protective system while attempting to locate a sewer service pipe at the bottom of a 7.3 foot deep trench.

29 CFR 1926.652(c)(2)(iii): Manufacturer's specifications, recommendations, and limitations, and manufacturer's approval to deviate from the specifications, recommendations, and limitations were not in written form at the jobsite during construction of the protective system:  Trench, 914 Pleasant Street, Amherst, MA: on or about March 14, 2019 manufacturer's specifications, regulations and limitations were not available in written form on the job site when the protective system was being constructed (assembled).  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
