29 CFR 1910.261(b)(1): The employer did not ensure that the main power disconnect switch or valve, or both, controlling its source of power or flow of material, was locked out or blocked off with padlock, blank flange, or similar device before conducting any maintenance, inspection, cleaning, adjusting, or servicing of equipment (electrical, mechanical, or other) that required entrance into or close contact with the machinery or equipment: (a) #5 High Density Tank Area - On or about March 19, 2019 employees were allowed to pull and repair CV- O8 valve without ensuring the system was lockout. Employees were exposed to burn hazards.
