29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    (a)  Jose Cardas Jimenez - 601 North 16th Street, Harrisburg, PA jobsite - The employees removing the shingles from the roof were exposed to a fall of approximately twenty two feet and were not using personal fall arrest systems, on or about March 27, 2019.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) Jose Cardas Jimenez - 601 North 16th Street, Harrisburg, Pa jobsite - The employer had not provided fall protection training to any of the employees removing shingles from the roof, on or about March 27, 2019.  ABATEMENT NOTE  As part of the employee training program, the employer shall maintain a written certification record which includes; employee name, date of the training, and signature of the person who conducted the training or the signature of the employer.  If the employer is relying in training conducted by another employer, the training record shall contain the date when the current employer determined that the employee was adequately trained.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
