29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    The employer is failing to conduct annual periodic reviews of the written specific procedures used as part of the company lockout and tag out (LOTO) program where employees are exposed to electrical shock, caught in, and amputation hazards.  This was most recently evident during an inspection conducted on March 27, 2019 at the facility located at 13748 F St., Omaha, NE 68137.  LOTO is required while employees are performing maintenance on various pieces of equipment such as cutters, joggers, lift tables, and processors.  The employer has machine specific written procedures as part of the written LOTO program; however, the procedures have not been formally reviewed within the past year as outlined in this subpart.

This was most recently evident during an inspection conducted on March 27, 2019 at the facility located at 13748 F St., Omaha, NE 68137.  The employer has employees working on various machines without properly training them on the standard operating procedures for using lock-out/tag-out (LOTO) to control the release of stored energy.

29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures:    The employer and outside employer failed to share their respective lock-out/tag-out (LOTO) procedures with each other; therefore, exposing employees to amputation and caught-in hazards.  This was most recently evident during an inspection on March 27, 2019 at the facility located at 13748 F St., Omaha, NE 68137.  The employer had an outside contractor come in to repair the dock leveler.  The employer did not inform the outside contractor of their LOTO procedures nor were they made aware of the outside contractors procedures.

This was most recently evident during an inspection on March 27, 2019 at the facility located at 13748 F St., Omaha, NE 68137.  The company utilizes outside contractors to perform maintenance on the dock leveler whereas the contractors utilize their own LOTO procedures.  The employer has not trained its employees to understand those LOTO procedures.
