29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures:     The employer and outside employer failed to share their respective lock-out/tag-out (LOTO) procedures with each other; therefore, exposing employees to struck-by, amputation, and caught-in hazards.  This was most recently evident during an inspection on March 27, 2019 at the facility located at 13748 F St., Omaha, NE 68137.  The employer was contracted to repair a dock leveler and neither of the employers informed each other of their respective LOTO procedures.
