On or about March 28, 2019, in a 12 foot deep excavation, employees were not protected from cave-in from the walls by the use of a protective system.

29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary:  a. On or about March 29, 2019, employees were working in a 12-foot deep excavation, where the spoil pile was placed on the leading edge of the excavation, exposing employees to being struck by the material.
