29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a.  On or about March 26, 2019, at the above addressed jobsite, employees used a JLG 860SJ aerial lift model number 768166 to access the roof and were not trained to recognize the hazards associated with using the aerial lift. The employees were thereby exposed to the hazards associated with falls.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a. On or about March 26, 2019, at the above addressed jobsite, employees performing siding trimming operations from the roof, were exposed to falls of approximately one (1) story, created by the presence of unprotected sides.

On or about March 26, 2019, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
