29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.   On or about March 27, 2019, a Hyd-Mech model S-20 band saw was not guarded, in that the unused portion of the saw blade was not protected. Operators were exposed to contact hazards and increased risk for laceration during use, such as when cutting one and a half inch pipe for the handrail job.

29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall:  29 CFR 1910.132(d)(1)(i): Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment;  On or about March 27, 2019, the employer failed to select and require the use of PPE to protect employees from burns on the arms and torso related to welding activities. The employer did not select protective equipment and did not require protective equipment use for welding activities, such as but not limited to, above the shoulder and overhead position welding which involved increased risk of thermal burns from welding spatter during routine duties.

29 CFR 1910.305(g)(1)(iv): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:  29 CFR 1910.305(g)(1)(iv)(A): As a substitute for the fixed wiring of a structure;  On or about March 27, 2019, the employer substituted multiple connected extension cords in lieu of fixed wiring in the building 2 production areas that occupied the west side of the building and areas near the Ficep CNC machine. Due to this condition, employees were at increased risk for electric shock hazards.
