29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:  a) Jobsite: On January 17, 2019 and at times prior thereto an employee performed soffit work from an A-frame ladder.  The employee did not work within the side rails of the ladder or maintain three points of contact with the ladder. The employee had one foot on the ladder, one foot on a 2x4 wood guardrail, holding onto a truss above and swinging a hammer.    Note) Certification of corrective action is required for this item.
