29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    a)  On or about April 1, 2019, in the 12500 block of Texas Highway 3, employees working from the roadway at night did not use Class 3 reflective clothing, exposing employees to struck by hazards.

29 CFR 1910.268(d)(1): Before work was begun in the vicinity of vehicular or pedestrian traffic which could endanger employees, warning signs, flags, barriers, and/or other traffic control devices were not placed conspicuously to alert and channel approaching traffic:    a)  On or about April 1, 2019,  in the 12500 block of Texas Highway 3, warning signs, flags, barriers and other traffic control devices were not used to alert and channel approaching traffic at all entry points.
