29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):    On or about 4/2/2019, at 1301 Progress Center Avenue, Lawrenceville, GA: Where employee(s) operated powered industrial truck near other workers the following but not limited to safety defects and deficiencies were observed; damage and exposed live conductor battery wiring, non-operating light(s), damage hydraulic lines, seatbelt not functioning properly, and broken fork pin lift handles. The employer failed to remove the powered industrial truck from service until the repairs could be made.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    On or about 4/2/2019, at 1301 Progress Center Avenue, Lawrenceville, GA: Employees are exposed to eye and face injuries/hazard where:    a) They fill the low or empty battery cells filled with water exposing them to the corrosive Sulfuric Acid chemical.  b) They fill change out the propane tanks on the powered industrial trucks exposing them to burns similar to frostbite from the hazardous chemical.    The Woodgrain Distribution was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.133(a)(1), which was contained in OSHA inspection number 986879, citation number 1, item number 2a and was affirmed as a final order on 10/1/2014, with respect to a workplace located at 6280 Best Friend Road, Suite 400, Norcross, GA 30071.
