29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    A) Black Creek Construction LP on or about April 2, 2019 at the address of 5526 Barred Owl, Sylvania, OH: The employer did not ensure employees were protected from falls while installing sheeting materials (OSB) from atop a new residential type structure.  The employees were exposed to a potential 10 foot and 19 foot falls to the level below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  A) Black Creek Construction LP on or about April 2, 2019 at the address of 5526 Barred Owl, Sylvania, OH: The employer had not trained employees in the appropriate use of fall protection, such as but not limited to, the use of personal fall arrest systems while performing various roofing operations. The employees did not recognize the hazards associated with working from an elevated locations of  (19 and 10 feet) without the benefit of fall protection.
