29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) Performance Gutters, Inc. at 1416 Garfield Ct. Louisville, CO 80027: On or before 4/2/19 the employer did not protect employees from injury in that an employee installing gutters on the roof of a two story single family home was not wearing fall protection. This condition exposed the employee to a fall hazard of approximately 10'.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:    (a) Performance Gutters, Inc. at 1416 Garfield Ct. Louisville, CO 80027: On or before 4/2/19 the employer did not protect employees from injury in that the top of a portable step ladder was used to access the roof by employees installing new gutters. This condition exposed employees to fall hazards.
