29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a.    On or about April 2, 2019, at the scaffold in front of the building, on the second level, the platform was only planked for 18-inches of the 57 inches between the uprights, exposing employees to 10-feet and 4-inches fall hazard.  b.   On or about April 2, 2019, at the scaffold in front of the building, on the third level, the platform was only planked for 18-inches of the 28-inches between the uprights, exposing employees to a 14 feet fall hazard.

29 CFR 1926.451(e)(6): Integral prefabricated scaffold access frames did not meet the requirements of paragraphs (e)(6)(i)-(vi) of this section:  a.    On or about April 2, 2019, the rungs of the access-integrated fabricated scaffold end frames were used as horizontal supports for the 3rd level platform planking, and there was no other means of access to that scaffold level.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a.  On or about April 2, 2019, at the front of the building, on the second level of the scaffold, 3 employees were exposed to a 10-feet and 4-inches fall hazard in that they were not protected by a fall protection system.  b.  On or about April 2, 2019, at the front of the building, on the third level of the scaffold, 3 employees were exposed to a 14-feet fall hazard in that they were not protected by a fall protection system.  c.  On or about April 2, 2019, at the front of the building, on the pickboard stage plank, 3 employees were exposed to a 14 feet and 6-inches in that they were not protected by a fall protection system.

On or about April 2, 2019, on the 6:12 pitch front slope of the first-story roof, 3 masonry employees were exposed to an 8-feet fall hazard from the eave, in that they were not protected by a fall protection system.
