29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about April 2, 2019, at a worksite located at Quail Creek Crossing, 873 Quail Creek Blvd., Stow, OH 44224, during residential framing activities employees were exposed to fall hazards of approximately 9-10 feet. The employer failed to ensure fall protection systems/devices were used by all employees working on the roof at all times.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:   On or about April 2, 2019 during residential construction, the employer did not have a safety and health program implemented at the worksite. The employer did not ensure employees were provided safety and health training; including fall protection or that there was a system in place to recognize and correct unsafe working conditions.
