29 CFR 1926.20(b)(2):    Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    On 4/2/2019, the general contractor, Digital Roofing Innovations, Inc. was watching the sub-contractors installing the roof of a commercial building located at 12001 North Forty Drive, Saint Louis, Missouri, 63131.  The general contractor or controlling employer did not conduct an inspection of the job site, materials, and equipment.  The following hazardous conditions existed:         a)  Eight workers from Flanagan Construction, Inc. were on the roof of a commercial building approximately 13 feet            above the lower level without the use of a fall protective system.       b)  Digital Roofing Innovations, Inc. rented a skid steer from Sunbelt Rentals and allowed sub-contractor employees to use            the equipment without proper training.       c)  Sub Contractor employees were exposed within inches to an electrical drop line carrying equal or greater than            480 volts.
