29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling  a) Nicholson de-barker machine: On April 4, 2019 and at times prior thereto employees were exposed to a fall greater than 8 feet to the lower level and to the ground below when performing scheduled and unscheduled maintenance on and around the de-barker "HOG" machine.  A guardrail system was not in place around the upper work platforms.    b) Shaker machine roof:  On April 4, 2019 and at times prior thereto employees were exposed to an estimated 10 to 20 foot fall to the ground below when performing scheduled and unscheduled maintenance on the shaker machine.  A guardrail system was not in place around the upper work platform, or the area used for access to the platform.      Note) Corrected during inspection.

29 CFR 1910.29(b)(2): (2) Midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members were not installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high:  a) Chipper house: On April 4, 2019 and at times prior thereto a mid-rail was missing from the guardrail system around a floor opening in the corner of the chipper room. 29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was isolated from the energy source and rendered inoperative:    a) Facility:  On April 4, 2019 and at times prior thereto, the employer had an informal, lock-out system for the de-barker "HOG" electrical energy sources in the motor control center room.  However, the employer had not developed, documented or implemented a formal energy control program that would have specific procedures/techniques, a periodic review and formalized employee training.  Employees perform scheduled and un-scheduled maintenance on machines including but not limited to the de-barker and chipper.      Note) Certification of abatement AND documentation as supporting evidence of corrective action are required.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts or flying chips:   a) De-barker machine: On April 4, 2019 and at times prior thereto, a complete guard was not in place on the tail spool of waste chain conveyor, exposing employees to an in-running nip point.  b) Chipper out-feed: On April 4, 2019 and at times prior thereto, a guard was not in place on the tail spool of the chipper out-feed, exposing employees to rotating parts.    c) Blower Shaft: On April 4, 2019 and at times prior thereto, a guard was not in place around a rotating shaft, exposing employees to the rotating part.   d) Re-chipper: On April 4, 2019 and at times prior thereto, a guard was not in place on  a rotating shaft, exposing employees to rotating parts.   Note) Corrected during inspection.
