Half-face respirators used by Powder Coat Painter employees were hung on hooks in the area around the Gema powder coat booth, which exposed them to damage, contamination and dust when not in use.

29 CFR 1910.147(d):  The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    At a facility located at 1608 Morrison Road, Rothschild, WI 54474; employees were exposed to machine struck-by and caught-between hazards associated with the moving ventilation baffles of the Gema powder coat booth. The employer did not ensure that energy control application steps to control the electrical and pneumatic energy sources were implemented prior to allowing employees to enter and clean the powder coat booth. The energy to the baffles was not shut down or turned off prior to engaging in this servicing and maintenance activity.    As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.1000(a)(2):  An employee's exposure to any substance in Table Z-1, the exposure limit of which is not preceded by a "C", shall not exceed the 8-hour Time Weighted Average given for that substance any 8-hour work shift of a 40-hour work week:    At a facility located at 1608 Morrison Road, Rothschild, WI 54474; employees were overexposed when:    a) On June 27, 2019, a Powder Coat Painter employee located at the entrance of the Gema powder coat booth was exposed to an 8-hour time-weighted average (TWA) of particulates not otherwise regulated (PNOR) at a level of 24.3 mg/m^3. This exceeded the Table Z-1 exposure limit of 15 mg/m^3 as an 8-hour TWA. The sampling was performed for 429 minutes during one shift. An exposure level of 0 was assumed for the unsampled 51 minutes.    b) On June 27, 2019, a Powder Coat Painter employee located at the exit of the Gema powder coat booth was exposed to an 8-hour time-weighted average (TWA) of particulates not otherwise regulated (PNOR) at a level of 22.6 mg/m^3. This exceeded the Table Z-1 exposure limit of 15 mg/m^3 as an 8-hour TWA. The sampling was performed for 468 minutes during one shift. An exposure level of 0 was assumed for the unsampled 12 minutes.

29 CFR 1910.1000(e): To achieve compliance with paragraphs (a) through (d) of this section, administrative or engineering controls must first be determined and implemented whenever feasible. When such controls are not feasible to achieve full compliance, protective equipment or any other protective measures shall be used to keep the exposure of employees to air contaminants within the limits prescribed in this section. Any equipment and/or technical measures used for this purpose must be approved for each particular use by a competent industrial hygienist or other technically qualified person. Whenever respirators are used, their use shall comply with 1910.134:  At a facility located at 1608 Morrison Road, Rothschild, WI 54474; feasible administrative and/or engineering controls were not determined and implemented in order to reduce Powder Coat Painter employees' exposures to concentrations of particulate not otherwise regulated (PNOR) to limits prescribed in 1910.1000(a) Table Z-1.  Applicable administrative and/or engineering controls may include, but are not limited to the following:  1) Review the installation of the Gema powder coat booth with the manufacturer to ensure; i) the ventilation system is optimally capturing dust, and ii) Powder Coat Painter employees are located and operating at their ideal locations/positions. Determine with the manufacturer if any other engineering or administrative controls could increase capture efficiency and reduce Powder Coat Painter employees' exposures.  2) Conduct airflow studies in conjunction with personal air monitoring to determine effectiveness of the Gema powder coat booths dust collection system and ideal working positions for the Powder Coat Painter employees to limit their exposure.    3) Provide Powder Coat Painter employees with additional powder coat tools to put distance between the painters and the powder coat. Evaluate the use of elongated or other specialty-style powder coat guns.  4) Through training and evaluation, ensure Powder Coat Painters employees do not place themselves in positions that locates their breathing zones between the work piece and powder coat booth ventilation system.
