29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:      a) On or about 04/04/19, on the site of a 3-story house at 118 19th St., Buffalo, NY: Two employees engaged in roofing work in various areas on the steep roof were exposed to a fall of up to approx. 25 ft.  No fall protection system was being used. Pitch of the roof was approx. 8 in 12.       NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:      a) On or about 04/04/19, on the site of a 3-story house at 118 19th St., Buffalo, NY: A Little Giant type of ladder was set up on the ground on the right side of the house and used by employees to access & exit the porch roof. The side rails of that ladder only extended approx. 22 in. above the porch roof.      b) On or about 04/04/19, on the site of a 3-story house at 118 19th St., Buffalo, NY: An extension ladder was set up on the left side of the porch roof and used by employees to access & exit the upper roof. The side rails of that ladder only extended approx. 25 in. above the upper roof.        NO ABATEMENT CERTIFICATION REQUIRED
