29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights.   On or about April 3, 2019, at the Ruston Sports Complex, the employer failed to ensure that the scaffolding was fully planked. The scaffold had three boards where five were required. Employees used the scaffold system in order to stock bricks.

The second section of the scaffold rear scaffold pole was not equipped with a base plate or mud sill. Employees used the scaffold system to stock masonry material.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.   On or about April 3, 2019 at Ruston Sports Complex, Grand Stand Bleachers Section B 4 the employer failed to ensure that the scaffold system was complete with a ladder. Employees used the scaffold system as a means of access and egress.
