29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) For the employees laying stone on or about April 8, 2019 and times prior thereto, the employer did not fully deck the adjustable Morgen scaffold at the northeast of the building when working at heights greater than 10 feet above the ground.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) For the employees working on the adjustable tower scaffold on or about April 8, 2019 and times prior thereto, the employer did not provide adequate fall protection when working greater than 10 feet from the ground. b) For the employee working from the out rigger attached to the scaffold towers on or about April 8, 2019 and times prior thereto, the employer did not provide fall protection when working greater than 10 feet from the ground.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. For the employees working on the tower scaffold on or about April 8, 2019 and times prior thereto, the employer did not train employees on the safe use of the scaffold prior to allowing employees to work from the scaffold that was greater than 10 feet above the ground.
