29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    (a) 115 Dashem Lane Millheim PA - An employee was observed performing roofing work activity on an 1 in 12 pitch residential porch roof without the benefit of a fall protection system, thereby exposing employees to a fall of approximately 9 feet 6 inches to the ground below, on or about April 10, 2019.    (b) 115 Dashem Lane Millheim PA - An employee was observed performing roofing work activity on an 1 in 12 pitch residential second porch roof without the benefit of a fall protection system, thereby exposing employees to a fall of approximately 8 feet 8 inches to the ground below, on or about April 10, 2019.    (c) 115 Dashem Lane Millheim PA - Employees were performing roofing work activity on an 8 in 12 pitch residential main roof without the benefit of a fall protection system, thereby exposing employees to a fall of approximately 18 feet high to the ground below, on or about April 10, 2019.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:    (a) 115 Dashem Lane Millheim PA  - Employee(s) were using a eight foot Werner stepladder and stepped on the top step of the ladder to gain access to the back porch roof of the residential home , thereby exposing employees to an 8 foot 8 inch fall, on or about April 10, 2019.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
