29 CFR 1910.132(d)(1)(i): The employer did not have each affected employee use the type(s) of PPE that will protect the affected employee from the hazards identified in the hazard assessment:  On or about April 6, 2019 a worker was not wearing a face shield while opening a valve to the sodium hydroxide tank when the valve failed spraying the caustic solution into his eyes and face.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  On or about April 6, 2019 a worker was sprayed with sodium hydroxide and did not have an eye wash facility within a reasonable distance to flush the chemical from his eyes and face.
