29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:     (a)  Jobsite:  On or about April 9, 2019, an employee (who was not tied off) was standing on the mid-rail of the aerial lift.  One leg was on the outside of the basket and one leg was on the inside of the aerial lift.  The fall hazard to the employee was about 14 feet.
