The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck:  The employer is failing to protect employees from a struck by hazard when operating an all-terrain forklift at a residential construction site. This was most recently documented on or about April 16, 2019 at 1630 La Plata Drive, Lincoln, NE 68521 when an employee was operating an all-terrain forklift and he had not received any training.

This was most recently documented on or about April 9, 2019 at a residential home construction jobsite located at 1630 La Plata Drive, Lincoln, NE 68521 where the employer has employees using pneumatic nail guns during framing operations without wearing any ANSI approved eye protection.

This was most recently documented on or about April 16, 2019 at 1630 La Plata Drive, Lincoln, NE 68521 when employees were observed installing trusses on a residential structure while working at greater than 6 feet above the ground without fall protection.

The employer is failing to provide documentation of fall protection training. This was most recently documented on or about April 9, 2019 at 1630 La Plata Drive, Lincoln, NE 68521. This should include training on mitigating those hazards through an acceptable method of fall protection.
