29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about 4/08/2019, Stallion Restoration Inc. was removing and installing roofing materials on a roof of a residential building located at 1365 Caves Springs Estate Drive in St. Peters, MO. An employee working from ladder jack scaffolding and an employee working over the garage were not wearing protective helmets while working beneath two employees above them. The employees above them were removing roofing materials; therefore, the two employees below them were subject to head injuries from possible falling tools and materials.

On or about 4/08/2019, Stallion Restoration Inc. was removing and installing roofing materials on a roof of a residential building located at 1365 Caves Springs Estate Drive in St. Peters, MO. An employee was installing roofing materials from improperly constructed ladder jack scaffolding. The ladders were not secured at the top, the planks were bridged and one end of a plank was setting on top of the garage roof. The scaffolding was not designed or constructed by a qualified person and loaded in accordance with that design.

On or about 4/08/2019, Stallion Restoration Inc. was removing and installing roofing materials on a roof of a residential building located at 1365 Caves Springs Estate Drive in St. Peters, MO. An employee was installing roofing materials from improperly constructed ladder jack scaffolding. The ladder jack scaffolding consisted of two ladders and three platforms. The platforms were bridged together on the center ladder. One plank was supported by the two ladders and the other plank was bridged over a ladder on one side and placed on the garage roof on the other end.
