29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  a)  Job site - on or about April, 10, 2019, employees were required to access and performed work in a trench 8 to 12-foot deep trench. The employer failed to ensure the trench had adequate cave-in protection, thereby exposing employees to crushed-by and engulfment hazards.
